ABSTRACT

Objetivo: avaliar e comparar o conteúdo energético do leite humano cru e do leite humano processado de recém-nascidos pré-termo. Método: foram coletadas 68 amostras de leite por meio de ordenha manual e o processamento foi realizado no Banco de Leite Humano. O valor energético foi obtido por meio de cálculos matemáticos específicos. Resultados: foram analisadas 32 amostras de leite humano cru e 28 de leite humano pasteurizado. O percentual médio de creme foi de 3,84±1,3% e mediana de 4,5 % no leite pasteurizado, e no cru de 8,9±4,6% e 8,53%. A média e mediana do conteúdo energético do leite cru foi de 78,91±15,46 kcal/100 ml e 81,07 kcal/100 ml e no leite pasteurizado 65,18±9,67 kcal/ 100 ml e 61,8 kcal/100 ml. Conclusão: o leite humano cru possui percentual de gordura e conteúdo energético maior do que o pasteurizado, mas este ainda é recomendado devido seus benefícios protetores.


Objective: to evaluate and compare the energy content of raw human milk and processed human milk from preterm newborns. Method: 68 milk samples were collected by hand milking and processing was carried out at the Human Milk Bank. The energy value was obtained through specific mathematical calculations. Results: 32 samples of raw human milk and 28 of pasteurized human milk were analyzed. The average percentage of cream was 3.84±1.3% and a median of 4.5% in pasteurized milk, and in raw milk, 8.9±4.6% and 8.53%. The mean and median energy content of raw milk was 78.91±15.46 kcal/100 ml and 81.07 kcal/100 ml and in pasteurized milk 65.18 ± 9.67 kcal/100 ml and 61.8 kcal/100 ml. Conclusion: raw human milk has a higher percentage of fat and energy content than pasteurized milk, but it is still recommended due to its protective benefits.


Objetivo: evaluar y comparar el contenido energético de la leche humana cruda y la leche humana procesada de recién nacidos prematuros. Método: Se recolectaron 68 muestras de leche por ordeño manual y el procesamiento se realizó en el Banco de Leche Humana. El valor energético se obtuvo mediante cálculos matemáticos específicos. Resultados: se analizaron 32 muestras de leche humana cruda y 28 de leche humana pasteurizada. El porcentaje medio de nata fue 3,84±1,3% y una mediana de 4,5% en leche pasteurizada, y en leche cruda 8,9±4,6% y 8,53%. El contenido energético medio y mediano de la leche cruda fue de 78,91±15,46 kcal/100 ml y 81,07 kcal/100 ml y de la leche pasteurizada de 65,18±9,67 kcal/100 ml y 61,8 kcal/100 ml. Conclusión: la leche humana cruda tiene un mayor porcentaje de contenido graso y energético que la leche pasteurizada, pero aun así es recomendada por sus beneficios protectores.

ABSTRACT

Resumen La pandemia de enfermedad por coronavirus 2019 (COVID-19) ha afectado a todas las dimensiones de la atención en salud, entre ellas el aseguramiento de la lactancia materna exclusiva y su promoción. El riesgo de contagio y las consecuencias de la pandemia han provocado preocupación entre las futuras madres o las que se ya encuentran lactando debido al riesgo de una posible transmisión del virus a través de la leche materna. Aunque aún no se ha detectado el coronavirus 2 del síndrome respiratorio agudo grave (SARS-CoV-2) activo en la leche materna. El miedo al contagio ha favorecido las políticas de aislamiento madre-hijo. Hasta el momento no existe evidencia de transmisión vertical y el riesgo de transmisión horizontal en el lactante es similar al de la población general. En lactantes con COVID-19 la lactancia materna incluso puede cambiar favorablemente el curso clínico de la enfermedad.


Abstract The COVID-19 pandemic has affected the health attention in all dimensions, one of them, the exclusive breastfeeding assurance and her promotion. The high risk of contagion and the pandemic consequences have raised a number of concerns in future mothers or those who are breastfeeding because of the risk of a possible transmission of the virus through breast milk. Although SARS-CoV2 has no evidence of being active on breast milk, the fear of contagion has favored mother-child isolation policies. At this point, there are no evidence of vertical transmission and the risk of horizontal transmission in the infant is similar to the general population. Breastfeeding in newborn with COVID-19, can even favorably change the clinical course of the disease.

ABSTRACT Objective: To analyze the composition of macronutrients present in the milk of mothers of preterm newborn infants (PTNB) - protein, fat, carbohydrate, and calories - by gestational age (GA), chronological age (CA) and maternal variables. Methods: Longitudinal study that analyzed 215 milk samples from the 51 mothers of PTNB admitted in three Neonatal Intensive Care Units of Rio de Janeiro from May/2013-January/2014. Milk collection was performed by pickup pump, on a fixed day of each week until discharge. The spectrophotometric technique with Infrared Analysis (MilkoScan Minor 104) was used for the quantitative analysis. A sample of 7 mL of human milk was taken from the total volume of milk extracted by the mother. The data was grouped by GA (25-27, 28-31, 32-36, 37-40 weeks) and by CA (zero to 4, 5-8, 9-12, 13-16 weeks). Results: Protein, carbohydrate, fat and calories did not show any pattern of change, with no difference among groups of GA. When the macronutrients were analyzed by groups of CA, protein decreased, with significant difference between the first two groups of CA. Carbohydrates, fat and calories presented increasing values in all groups, without significant differences. Weight gain during pregnancy, maternal hypertension and maternal age were associated with changes in fat and calories in the first moment of the analysis of milk. Conclusions: There was a significant decrease in the levels of protein during the first eight weeks after birth. CA may be an important factor in the composition of human milk.


RESUMO Objetivo: Analisar a composição dos macronutrientes presentes no leite de mães de recém-nascidos pré-termo (RNPT) - gorduras, carboidratos e calorias - por idade gestacional (IG), idade cronológica (IC) e variáveis maternos. Métodos: Estudo longitudinal para analisar 215 amostras de leite de 51 mães de RNPT admitidos em três unidades neonatais do Rio de Janeiro de maio/2013 a janeiro/2014. A coleta de leite foi realizada por bomba coletora, em dia fixo a cada semana até a alta. Utilizou-se a técnica espectrofotométrica com análise de infravermelho (MilkoScan Minor 104) para a análise quantitativa. Uma amostra de 7 mL de leite humano foi retirada do volume total de leite extraído pela mãe. Os dados foram agrupados por IG (25-27, 28-31, 32-36 e 37-40 semanas) e por IC (0-4, 5-8, 9-12 e 13-16 semanas). Resultados: Proteínas, carboidratos, gorduras e calorias não apresentaram nenhum padrão de mudança, não havendo diferença entre os grupos de IG. Quando os macronutrientes foram analisados por grupos de IC, a proteína diminuiu, com diferença significante entre os dois primeiros grupos de IC. Carboidratos, gorduras e calorias apresentaram valores crescentes em todos os grupos, sem diferença estatística. O ganho de peso durante a gestação, a presença de hipertensão arterial e a idade materna foram associados a alterações de gordura e calorias no primeiro momento da análise do leite. Conclusões: Observou-se redução estatisticamente significante nos níveis de proteína durante as primeiras oito semanas após o nascimento. A IC pode ser um fator importante na composição do leite humano.

ABSTRACT Objective: To evaluate the effect of pasteurization on antioxidant and oxidant properties of human milk. Methods: 42 samples of milk before and after pasteurisation were used to evaluate the antioxidant activity by the ferric reducing capacity and by scavenging the 2,2'-azino-bis 3-ethylbenzthiazoline-6-sulfonic acid radical. Lipid peroxidation was estimated by the concentration of malondialdehyde product using the thiobarbituric acid reactive substances assay and by the evaluation of advanced oxidation protein products. Results: No significant difference was observed in fresh human milk and after pasteurization in relation to antioxidant properties determined by the ferric reducing capacity (50.0±3.4% and 48.8±3.0%, respectively) and by scavenging the 2,2'-azino-bis 3-ethylbenzthiazoline-6-sulfonic acid radical (28.9±1.5% and 31.2±1.3%, respectively). The results of malondialdehyde (62.6±4.1 and 64.3±3.6 µM/mg) and protein oxidation products (59.4±3.4 and 54.2±3.8 µM/L) of fresh and pasteurized milk, respectively, did not exhibited any significant difference. Conclusions: This data showed that human milk has an important antioxidant activity and that the pasteurizing process does not influence the antioxidant capacity, avoiding the peroxidation of breast milk lipids and the formation of advanced protein oxidation products.


RESUMO Objetivo: Avaliar o efeito da pasteurização nas propriedades antioxidantes e oxidantes do leite humano. Métodos: Foram utilizadas 42 amostras de leite cru e após a pasteurização, para avaliação da atividade antioxidante pelos métodos da capacidade redutora do ferro e sequestro dos radicais derivados do ácido 2,2'-azino-bis (3-etilbenzotiazolina-6-sulfônico). A peroxidação lipídica (PL) foi estimada pela determinação das substâncias reativas ao ácido tiobarbitúrico e pela avaliação dos produtos proteicos de oxidação avançada. Resultados: Não se observou diferença significante no leite humano cru nem após a pasteurização em relação às propriedades antioxidantes determinadas pelo método da redução do ferro (50,0±3,4% e 48,8±3,0%, respectivamente) e pelo sequestro dos radicais derivados do ácido 2,2'-azino-bis (3-etilbenzotiazolina-6-sulfônico) (28,9±1,5% e 31,2±1,3%, respectivamente). Os resultados médios de malondialdeído [MDA] (62,6±4,1 e 64,3±3,6 µM/mg) e produtos de oxidação proteica (59,4±3,4 e 54,2±3,8 µM/L) entre os grupos leite fresco e leite pasteurizado, respectivamente, não evidenciaram diferença significante. Conclusões: Os dados demonstraram que o leite humano possui importante atividade antioxidante e que o processo de pasteurização não interfere nessa propriedade, evitando assim a peroxidação dos lipídios e a formação de produtos avançados de oxidação proteica.

ABSTRACT Objective: To evaluate the relationship between the maternal diet quality and the fatty acid composition of breast milk in the first trimester of lactation. Methods: This is an observational cross-sectional epidemiological study of nursing mothers. The data collection instruments were a semi-structured questionnaire for sample characterization and a recall of usual intake. Diet quality was assessed based on the healthy eating index (HEI). Samples of mature breast milk were obtained by hand milking. Milk fat was extracted using the Bligh-Dyer method and methylated with 0.25 mol/L sodium methoxide in methanol diethyl ether. A gas chromatograph equipped with a flame ionization detector determined the milk fatty acid profile. Pearson's and Spearman's correlation tests evaluated association between the variables. Subsequently, a multiple linear regression model was built and multivariate regression analysis was applied. Results: Our findings revealed an inverse relationship between the consumption of total fruits and the polyunsaturated fatty acid profile and a direct association of the intake of total fruits and total grains with monounsaturated and saturated fatty acids. Conclusions: The results of this study suggest that maternal diet quality affects the fatty acid composition of breast milk.


RESUMO Objetivo: Avaliar a relação entre a qualidade da dieta materna e a composição do leite humano em ácidos graxos no primeiro trimestre de lactação. Métodos: Trata-se de um estudo epidemiológico observacional e transversal com nutrizes. Os instrumentos de coleta dos dados foram questionário semiestruturado para caracterização da amostra e o recordatório de ingestão habitual. A qualidade da dieta foi avaliada por meio do índice de alimentação saudável (IAS). Amostras de leite materno maduro foram obtidas por meio de ordenha manual. A extração da gordura do leite ocorreu mediante o método de Bligh-Dyer, e essa gordura foi metilada com metóxido de sódio 0,25 mol/L em metanol dietil éter. O perfil de ácidos graxos do leite foi determinado por um cromatógrafo a gás equipado com detector por ionização de chamas. Realizaram-se teste de correlação de Pearson e teste de Spearman para verificar associação entre as variáveis. Posteriormente se aplicaram o modelo de regressão linear múltiplo e a análise de regressão multivariada. Resultados: Foi possível observar relação inversa entre o consumo de frutas totais e o perfil de ácidos graxos poli-insaturados e relação direta entre o consumo de frutas totais e cereais totais e os ácidos monoinsaturados e saturados. Conclusões: Sugere-se que a qualidade da dieta reflete na composição de ácidos graxos do leite materno.

Abstract Objective: To identify whether the effects of thyroid disease during pregnancy and lactation affect the nutritional composition of human milk. Methods: Systematic review of the scientific literature using the Medical Literature Analysis and Retrieval System Online/MedLine databases to evaluate the association of thyroid diseases during pregnancy and lactation with the nutritional composition of human milk. There was no delimitation by period or by language, and the searches were completed in March 2019. The following descriptors were applied: human milk AND thyroid AND composition, using the preferred reporting items for systematic reviews and meta-analyses (PRISMA) protocol for data search, selection, and extraction. The flowchart proposed for bibliographic search resulted in 12 articles and, of these, four were selected. Results: The articles elected for this review were published between 1976 and 2018. Two studies found significant differences in the nutritional composition of mothers' milk with hypothyroidism or overweight compared with the milk of those without hypothyroidism. Studies have shown that the presence of the disease led to changes in the nutritional composition of human milk, especially a higher concentration of human milk fat. Conclusion: It is extremely important that these women have continuous nutritional follow-up to minimize the impact of these morbidities on the nutritional composition of human milk.


Resumo Objetivo: Identificar se os efeitos da doença da tireoide durante a gestação e lactação afetam a composição nutricional do leite humano. Métodos: Revisão sistemática da literatura científica por meio das bases de dados Medical Literature Analysis and Retrieval System Online/MedLine a fim de avaliar a associação das doenças da tireoide na gestação e na lactação com a composição nutricional do leite humano. Não houve delimitação por período nem por idioma, e as buscas foram finalizadas em março de 2019. Foram aplicados os seguintes descritores: human milk AND thyroid AND composition, utilizando protocolo preferred reporting items for systematic reviews and meta-analyses (PRISMA) para a busca, seleção e extração de dados. De acordo com o fluxograma proposto, a busca bibliográfica resultou em 12 artigos e, destes, quatro foram selecionados. Resultados: Os artigos elegidos para a presente revisão foram publicados entre 1976 e 2018. Dois estudos verificaram diferenças significativas na composição nutricional do leite de mães com hipotireoidismo ou excesso de peso em comparação ao grupo controle sem hipotireoidismo. Os estudos demonstraram que a presença da doença levava a modificações na composição nutricional do leite humano, principalmente em relação à maior concentração da gordura. Conclusão: É de extrema importância que essas mulheres tenham acompanhamento nutricional contínuo a fim de minimizar o impacto dessas morbidades sobre a composição nutricional do leite humano.

Objetivo: Comparar las concentraciones de los oligoelementos cobre y zinc en la leche materna, de vaca y de cabra. Métodos: Estudio descriptivo de corte transversal. Se recogieron muestras de leche materna de madres lactantes sanas (91 mujeres) que asistieron a un centro materno infantil en la ciudad de Maracaibo-Venezuela. Previo consentimiento informado se seleccionaron madres que cumplieron con los criterios de inclusión. Las concentraciones de los oligoelementos en estudio, se determinaron mediante espectrofotometría de absorción atómica con llama. Resultados: En el análisis de la muestra de leche humana se hallaron concentraciones de cobre de 0,290 ± 0,04 mg/L y concentraciones de zinc de 1,580 ± 0,35 mg/L. Estos valores son considerados aceptables según la ingesta diaria de referencia para estos oligoelementos y fueron más elevada en muestras en leche de cabra según la literatura, mientras que las concentraciones de zinc en ambas leches (cabra y vaca), registradas por otros especialistas, fueron más elevadas que en la leche humana del presente estudio. Conclusiones: Las concentraciones de cobre y zinc halladas en la leche materna en este trabajo son las ingestas recomendadas para el desarrollo y madurez del sistema nervioso central del neonato(AU)


Objective: To compare the concentrations of trace elements like copper and zinc present in breast, cow's and goat's milk. Methods: Descriptive, cross sectional study. Samples of breast milk were collected from healthy lactating mothers (91 women) who attended a maternal and children health center in Maracaibo city, Venezuela. Mothers were selected prior informed consent who met the inclusion criteria. The concentrations of the studied trace elements were determined by atomic absorption spectrophotometry with flame. Results: In the analysis of the human milk´s sample, there were found concentrations of Cu (0.290 ± 0.04 mg/L) and concentrations of Zn (1.580 ± 0.35 mg/L). These levels are considered acceptable according to the permitted intake of trace elements and according to the literature, those are higher in goat milk´s samples, while concentrations of Zn in both milks (goat and cow), registered by other specialists were higher than in the human milk´s of the present study. Conclusions: The concentrations of copper and zinc found in breast milk are the recommended daily intake for the development and maturity of the central nervous system of the newborns(AU)

OBJECTIVE@#To study the association of fatty acid composition in human milk with breast milk jaundice (BMJ) in neonates.@*METHODS@#A total of 30 full-term neonates who were admitted to the neonatal intensive care unit from October 2016 to October 2017 and were diagnosed with late-onset BMJ were enrolled as the BMJ group. Thirty healthy neonates without jaundice or pathological jaundice who were admitted to the confinement center during the same period of time were enrolled as the control group. Related clinical data were collected, including sex, mode of birth, feeding pattern, gestational age, birth weight, gravida, parity, and peak level of total serum bilirubin. Breast milk was collected from the mothers, and the MIRIS human milk analyzer was used to measure macronutrients (fat, protein, and carbohydrate) and calorie. Gas chromatography was used to analyze the content of different fatty acids in breast milk.@*RESULTS@#The control group had higher levels of macronutrients in human milk than the BMJ group, with significant differences in fat, dry matter, and calorie (@*CONCLUSIONS@#Some macronutrients and fatty acid composition in human milk may be associated with the pathogenesis of BMJ in neonates.

Abstract Objectives: to determine the fatty acid composition of mature milk of nursing mothers and its distribution according to some maternal variables. Methods: this is a cross-sectional observational epidemiological study based on the eva-luation of the fatty acid profile of mature human milk. Samples of mature breast milk were taken from 106 nursing mothers, by manual milking and who were after the 5th postpartum week. The milk fat extraction was carried out by using the Bligh and Dyer method and methy-lated with 0.25 mol/L sodium methoxide in methanol diethyl ether. The fatty acid of the milk profile was determined by a Gas Chromatograph equipped with a flame ionization detector. Results: among the saturated fatty acids, the highest values were observed for palmitic (C16:0), stearic (C18:0), myristic (C14:0) and lauric (C12:0) fatty acids, respectively. Among the monounsaturated fatty acids, there was a higher contribution of oleic (C18:1) and palmi-toleic (C16:1) fatty acids, respectively. The total essential fatty acids (linoleic and α-linolenic) was 14.94%. Conclusions: a low content of essential fatty acids in the breast milk of the nursing mothers was observed in the present study, which are important for infant growth and deve-lopment. We suggest the need to implement nutrition education strategies aimed for pregnant women and nursing mothers who should be advised to eat healthier foods.


Resumo Objetivos: determinar a composição em ácidos graxos do leite maduro de nutrizes e sua distribuição segundo algumas variáveis maternas. Métodos: trata-se de um estudo epidemiológico observacional, transversal realizado a partir da avaliação do perfil de ácidos graxos do leite humano maduro. Amostras de leite materno maduro foram obtidas de 106 nutrizes, a partir da 5ª semana pós-parto, por meio de ordenha manual. A extração da gordura do leite foi realizada através do método de Bligh e Dyer, e metiladas com metóxido de sódio 0,25 mol/L em metanol dietil - éter. O perfil de ácidos graxos do leite foi determinado por um Cromatógrafo a Gás equipado com detector por ionização de chamas. Resultados: dentre os ácidos graxos saturados, foram observados valores mais elevados para os ácidos graxos palmítico (C16:0), esteárico (C18:0), mirístico (C14:0) e láurico (C12:0), respectivamente. Entre os ácidos graxos mono-insaturados, verificou-se maior contribuição dos ácidos graxos oleico (C18:1) e palmitoleico (C16:1), respectivamente. O total de ácidos graxos essenciais (linoleico e α- linolênico) foi de 14,94%. Conclusões: foi observado baixo teor de ácidos graxos essenciais no leite materno das nutrizes do presente estudo, que são importantes para o crescimento e desenvolvimento do lactente, sugerindo-se a necessidade de implementação de estratégias de educação nutricional direcionadas a gestantes e nutrizes que devem ser orientadas a consumir alimentos mais saudáveis.

RESUMEN La leche humana materna tiene una composición única para proporcionar a los recién nacidos todos los nutrientes esenciales como proteínas, grasas, hidratos de carbono, minerales, vitaminas y otras substancias fisiológicamente activas, con el fin de suplir en cantidad y calidad las necesidades calóricas y nutricionales del lactante. El objetivo fue establecer las concentraciones totales de cobre y zinc en la leche materna en sus distintas etapas: calostro, de transición y madura, determinadas espectrométricamente y la influencia del estado nutricional de madres lactantes; estudio descriptivo transversal en madres entre 18 y 35 años, sanas y no fumadoras; los resultados evidencian una disminución en las concentraciones de cobre y zinc a medida que la lactancia va evolucionando, además, el promedio de índice de masa corporal de las madres en período de lactancia reflejaron sobrepeso. Concluimos que el contenido calórico de los oligoelementos de la leche materna es esencial para el crecimiento de los niños por los beneficios nutricionales e inmunológicos que proporciona, por lo que es importante que la madre se encuentre en un estado nutricional adecuado que permita suministrar los requerimientos adecuados al niño para su crecimiento y desarrollo.


ABSTRACT Human milk has a unique composition and provides newborns with all essential nutrients such as proteins, fats, carbohydrates, minerals, vitamins and other physiologically active substances, in order to supply in quantity and quality the caloric and nutritional needs of the infant. The objective of this study was to establish the total concentration of copper and zinc in breast milk in its different stages: colostrum, transitional and mature milk, determined spectrometrically and the influence of the nutritional status of nursing mothers. We conducted a descriptive cross-sectional study in healthy non-smoking mothers between 18 and 35 years of age. Lower concentrations of copper and zinc as lactation progressed were observed. The average of body mass index of nursing mothers reflected overweight. We concludes that the caloric content of the trace elements of breast milk is essential for the growth of children because of the nutritional and immunological benefits that it provides, so it is important that the mother has an adequate nutritional state that allows to supply the adequate requirements for the child to grow and develop.

Resumo O objetivo deste artigo é identificar os efeitos das morbidades maternas sobre a composição nutricional do leite humano. Revisão sistemática da literatura utilizando protocolo PRISMA para a busca, seleção e extração de dados. De acordo com o fluxograma proposto a busca bibliográfica resultou em 1.582 artigos e, destes, 14 foram selecionados. Os artigos elegidos para a presente revisão foram publicados entre 1987 e 2016. Nove estudos verificaram diferenças significativas na composição nutricional do leite de mães com diabetes mellitus, hipertensão arterial ou excesso de peso em comparação ao grupo controle. A maioria dos estudos demonstrou que a presença de doenças crônicas acarreta modificações na composição nutricional do leite humano. Os principais resultados evidenciaram menor concentração de lactose e gordura no leite de mulheres com diabetes mellitus. Quanto a hipertensão, foi verificado níveis mais elevados de proteína total no colostro e no leite maduro. Para o excesso de peso, foi observado maior teor de gordura e energia. É de extrema importância que essas mulheres tenham acompanhamento nutricional contínuo a fim de minimizar o impacto dessas morbidades sobre a composição nutricional do leite humano.


Abstract This paper aims to identify the effects of maternal morbidities on the nutritional composition of human milk. This systematic review of the literature employed a PRISMA's protocol for searching, selecting, and extracting data. The flowchart proposed for bibliographic search resulted in 1,582 papers, of which 14 were selected for this work. The papers selected for this review were published between 1987 and 2016. Nine studies found significant differences in the nutritional composition of the milk of mothers with diabetes mellitus, arterial hypertension, or overweight compared to the control group. Most studies have shown that the presence of chronic diseases leads to changes in the nutritional composition of human milk. The main results showed a lower concentration of lactose and fat in the milk of women with diabetes mellitus. Concerning hypertension, higher levels of total protein were found in colostrum and mature milk. A higher fat and energy content was observed in overweight mothers. It is imperative that these women have continuous nutritional monitoring to minimize the impact of these morbidities on the nutritional composition of breast milk.

ABSTRACT

A fim de garantir a inocuidade do leite humano (LH) ordenhado, os bancos de LH realizam a sua pasteurização lenta, eliminando, dessa maneira, 100 % dos micro-organismos patogênicos passíveis de estarem presentes. O objetivo desse trabalho foi avaliar a influência da pasteurização clássica realizada pelos hospitais sobre os parâmetros físico-químicos do LH. Analisou-se um total de 38 amostras de LH, divididas em pasteurizado (LHP) e não pasteurizado (LHNP), quanto à sua acidez, conteúdo energético, gordura, sólidos não gordurosos, densidade, proteína, lactose, cinzas, índice de crioscopia e pesquisa das enzimas peroxidase e fosfatase. Os resultados apresentaram uma diferença significativa (p < 0,05) para os teores de gordura e proteína, e presença da enzima peroxidase em 21,05 % das amostras de LHP, envolvendo problemas na homogeneização e na temperatura da pasteurização do LH.

Abstract To evaluate the effect of vitamin A supplementation in postpartum infants and women on serum retinol levels and breast milk. The databases Medline, PubMed, Lilacs and SciELO were consulted. The descriptors used were vitamin A, dietary supplement, child, postpartum period, infant and nutrition programs policies. Search found 7432 articles. After elimination of duplicity and application of eligibility criteria, 8 studies remained. All evaluated the effect of vitamin A supplementation on immediate postpartum, five studies used retinyl palmitate supplementation, one with retinyl palmitate and two did not specify the form of supplementation. Six studies evaluated colostrum and two included supplementation of children. It was found that supplementation in the puerperium increases the concentrations of serum retinol and breast milk, however, this result was in the short term and was relevant when the previous concentrations of the mother were low. When maternal serum concentrations are adequate, the retinol content in milk does not change, with little relevance for children. Further studies should be performed to evaluate the effect of megadoses supplementation on serum concentrations of children.


Resumo Avaliar o efeito da suplementação de vitamina A, em lactentes e mulheres no pós-parto, nos níveis de retinol sérico e no leite materno. Foram consultadas as bases de dados Medline, PubMed, Lilacs e SciELO. Os descritores utilizados foram: vitamin A, dietary supplement, child, postpartum period, infant e nutrition programs policies. A busca identificou 7432 artigos. Após eliminação da duplicidade e aplicação dos critérios de elegibilidade permaneceram 8 estudos. Todos avaliaram o efeito da suplementação de vitamina A no pós parto imediato, cinco estudos utilizaram a suplementação com retinil palmitato, um com palmitato de retinila e dois não especificaram a forma de suplementação. Seis estudos avaliaram o colostro e dois incluíram a suplementação de crianças. Encontrou-se que a suplementação no puerpério aumenta as concentrações de retinol sérico e do leite materno, no entanto, este resultado foi a curto prazo e foi relevante quando as concentrações prévias da mãe eram baixas. Quando as concentrações séricas maternas encontram-se adequadas, pouco se altera o teor de retinol no leite, tendo pouco relevância para as crianças. Mais estudos devem ser realizados para avaliar o efeito da suplementação com megadoses nas concentrações séricas de crianças.

Abstract Objective: To evaluate the quality of the human milk expressed at home and at a human milk bank. Methods: This a retrospective, analytical, and observational study, performed by assessing titratable acidity records and the microbiological culture of 100 human milk samples expressed at home and at a human milk bank, in 2014. For the statistical analysis, generalized estimating equations (GEE) and the chi-squared test were used. Results: When comparing the two sample groups, no significant difference was found, with 98% and 94% of the samples being approved among those collected at the milk bank and at home, respectively. No main interaction effect between local and titratable acidity records (p = 0.285) was observed, and there was no statistically significant difference between the expected and observed values for the association between the collection place and the microbiological culture results (p = 0.307). Conclusions: The quality of human milk expressed at home and at the milk bank are in agreement with the recommended standards, confirming that the expression of human milk at home is as safe as expression at the human milk bank, provided that the established hygiene, conservation, storage, and transport standards are followed.


Resumo Objetivo: Avaliar a qualidade do leite humano ordenhado em domicílio e no Banco de Leite Humano. Métodos: Estudo retrospectivo, feito por meio da avaliação dos registros da acidez titulável e dos resultados de cultura microbiológica de 100 amostras de leite humano ordenhado em domicílio e em um Banco de Leite Humano, no ano de 2014. Para análises estatísticas foram utilizadas as Equações de Estimação Generalizadas (Generalized Estimating Equations - GEE) e o teste Qui-quadrado. Resultados: Na comparação dos dois grupos de amostras, não foi detectada diferença significativa, sendo que 98% e 94% das amostras foram aprovadas entre as coletadas no Banco de Leite e em domicílio, respectivamente. Não foi observado efeito principal de interação entre local e grau de acidez titulável (p = 0,285) e não se constatou diferença estatisticamente significante entre os valores observados e esperados para associação entre local de coleta e o resultado da cultura microbiológica (p = 0,307). Conclusões: A qualidade do leite humano ordenhado em domicílio e no Banco de Leite Humano estão de acordo com o padrão preconizado, comprovando que a ordenha de leite humano em domicílio é tão segura quanto a ordenha no Banco de Leite Humano, desde que sejam seguidas as normas de higiene, conservação, armazenamento e transporte estabelecidas.

RESUMO Objetivo: Determinar e comparar as concentrações de eletrólitos e minerais no leite humano em três grupos: amostras analisadas antes e após pasteurização de lactantes doadoras a termo e amostra de leite cru colhida à beira do leito de mães de recém-nascidos pré-termo. Métodos: Estudo descritivo de corte transversal. Foram dosadas concentrações de cálcio (Ca), fósforo (P), magnésio (Mg), sódio (Na) e potássio (K) em amostras aleatórias de leite humano nos três grupos. As amostras foram analisadas por mineralização ácida assistida por radiação micro-ondas e posteriormente por espectrometria de emissão óptica com plasma indutivamente acoplado e expressas em mg/L, com cálculo de médias e desvio padrão. A comparação entre os grupos foi feita por análise de variância (ANOVA)/teste de Tukey. Nível de significância aceito de 5%. Resultados: Observou-se redução significante dos teores de Ca (259,4±96,8 vs. 217,0±54,9; p=0,003), P (139,1±51,7 vs. 116,8±33,3; p=0,004) e K (580,8±177,1 vs. 470,9±109,4; p<0,0001) após a pasteurização. As amostras de leite cru colhidas à beira do leito apresentaram teores estatisticamente mais elevados de Na (2 vezes) do que o leite a termo de doadora. Ca e P só atingiriam a ingestão recomendada pela European Society of Pediatric Gastroenterology, Hepatology and Nutrition se o leite materno fosse ofertado em volume de 60 mL a cada 3 horas. Concentrações de Mg não diferiram entre os grupos. Conclusões: Houve uma redução significativa de Ca, P e K nas amostras após pasteurização e os valores de Na no leite cru coletado à beira do leito foram superiores àqueles pré-pasteurização.


ABSTRACT Objective: To determine and compare the concentrations of electrolytes and minerals in three different types of maternal milk samples: term donor milk before pasteurization, term donor milk after pasteurization and raw milk of mothers of preterm newborns at bedside. Methods: Descriptive cross-sectional study. Concentrations of calcium (Ca), phosphorous (P), magnesium (Mg), sodium (Na) and potassium (K) were measured in random samples of three human breast milk groups. Samples were analyzed using acid mineralization assisted by microwave radiation and further analysis by inductively coupled plasma optical emission spectrometry. Concentrations were expressed in mg/L, described as mean and standard deviation. The one-way ANOVA and Tukey's post-test were applied to determine the variability between the means of each group. Significance level was set at 5%. Results: There was a significant reduction in the content of Ca (259.4±96.8 vs. 217.0±54.9; p=0.003), P (139.1±51.7 vs. 116.8±33.3; p=0.004) and K (580.8±177.1 vs. 470.9±109.4; p<0.0001) in donor maternal milk before and after pasteurization. Samples of raw milk presented higher contents of Na than the donated milk (twice). The elements P and Ca would only reach the daily intake levels recommended by the European Society of Pediatric Gastroenterology, Hepatology and Nutrition if at least 60 mL of milk could be offered every 3 hours. Mg levels were not different between the three groups. Conclusions: There was a significant reduction in Ca, P and K levels in samples after pasteurization. The Na value in raw milk, collected at bedside, was higher than in the samples of donor's milk before pasteurization.

Resumen Introducción. La leche materna es esencial para el desarrollo del ser humano, pero puede contener sustancias tóxicas provenientes de la contaminación ambiental, especialmente en las áreas mineras. Objetivo. Determinar la prevalencia de la contaminación con mercurio de la leche materna de mujeres lactantes residentes en los municipios con explotación minera de oro. Materiales y métodos. Se hizo un estudio transversal de 150 madres lactantes de cuatro municipios mineros de Antioquia (El Bagre, Segovia, Remedios y Zaragoza), a quienes se les hizo una encuesta sobre factores sociodemográficos, ocupacionales y ambientales relacionados con el mercurio, y se les tomaron muestras de leche materna, de orina y de cabello. Se calculó el promedio de la concentración de mercurio y las prevalencias municipales de contaminación. Resultados. El promedio de la concentración de mercurio en la leche materna fue de 2,5 (± desviación estándar 9,2) µg/L. La prevalencia de muestras de leche materna con niveles altos de mercurio fue de 11,7 %. Conclusión. En este estudio se evidencia un grave problema en las regiones mineras auríferas de Antioquia por el efecto de la contaminación con mercurio en sectores de la población más vulnerable.


Abastract Introduction: Breast milk is essential for human development, but it may contain toxics derived from environmental pollution, especially in mining areas. Objective: To assess the prevalence of mercury contamination in breast milk and factors associated with its transfer to nursing mothers living in municipalities with gold mining. Materials and methods: We conducted a cross-sectional study with 150 nursing mothers in four municipalities of Antioquia (El Bagre, Segovia, Remedios and Zaragoza) with a mining tradition. We surveyed these mothers to obtain information on their sociodemographic, occupational and environmental factors related to mercury, and we took breast milk, urine and hair samples. We calculated mercury level averages and the contamination prevalence per municipality. Results: Mercury average in breast milk was 2.5 (± standard deviation: 9.2) mg/L. The prevalence of mercury contamination in samples with a high level of mercury was 11.7%. Conclusion: This study shows that there is a serious problem of mercury contamination in gold mining regions of Antioquia, with significant involvement of the most vulnerable population.

Abstract This study investigated the effect of human milk, alone and associated with sucrose, in the cariogenicity of biofilms in a microcosm biofilm model and compared with the cariogenicity of sucrose and bovine milk. Microcosm biofilms were grown in enamel discs in 24-well plates. Six growth conditions were studied: DMM (chemically defined artificial saliva - negative control), DMM with 1% of sucrose (DMM+s) (positive control), human milk with DMM, human milk with DMM+s, bovine milk with DMM, and bovine milk with DMM+s. After 5 days, the outcome variables surface hardness change (%SHC), microbiological composition of biofilms, and pH of supernatant were analyzed. All groups had significantly lower hardness loss compared to the DMM group with 1% of sucrose. Human and bovine milk associated with sucrose showed higher hardness loss. The supernatant pH values after 6 hours of different treatments were similar for the groups sucrose and human milk associated with sucrose (p>0.05). After 18 hours at rest in pure DMM, an increase in the pH of the supernatant was observed. Higher values of total microorganisms count were found for sucrose and bovine milk groups compared to the group supplemented only by DMM. Bovine milk group showed greater amount of total aciduric microorganisms in comparison to human milk group. Within the limits of this study, it can be infered that both human and cow milks have some cariogenic potential, although differing from sucrose in terms of mineral loss.

Resumen: Objetivo: Asociar la ingesta de flavonoides y carotenoides con el nivel en leche materna del anión superóxido, como marcador de estrés oxidativo. Material y métodos: Durante el periodo 2013-2015 se estudió a 100 mujeres lactantes de Córdoba (Argentina), dentro los primeros seis meses posparto; se evaluaron sus datos sanitarios, ingesta alimentaria y nivel lácteo del anión con regresión logística múltiple. Resultados: La ingesta de flavonoides, carotenoides provitamínicos y carotenoides no provitaminas fue de 72 (61) mg/día, 1 813 (1657) µg/día y 5 427 (3664) µg/día, respectivamente. El anión se asoció con la ingesta de flavanoles (RM=1.081; IC95 1.001-1.167) y flavanonas (RM=1.025; IC95 1.001-1.048). No se observó este efecto con otros flavonoides ni con los carotenoides. Conclusiones: La ingesta de flavanoles y flavanonas aumenta el riesgo de oxidación láctea, lo cual es relevante para realizar recomendaciones dietéticas.


Abstract: Objective: To associate the intake of flavonoids and carotenoids with the breast milk level of superoxide anion, as an oxidative stress marker. Materials and methods: 100 women from Cordoba (Argentina), who breastfed within the first postpartum 6 months, were studied during the 2013-2015 period, by evaluating their sanitary data, food intake and anion level in milk with multiple logistic regression. Results: The intake of flavonoids, provitamin A carotenoids and non-provitamin carotenoids was 72 (61) mg/d, 1813 (1 657) µg/d y 5427 (3 664) µg/d, respectively. The anion was associated with the intake of flavanols (OR=1.081; CI95 1.001-1.167) y flavanones (OR=1.025; CI95 1.001-1.048). This effect was not seen with other flavonoids and carotenoids. Conclusions: Intake of flavanols and flavanones increases milk oxidation risk, which is relevant to develop diet recommendations.

RESUMO Objetivos: Determinar a concentração de alfa-tocoferol no leite materno em diferentes períodos de lactação e estimar o provável fornecimento de vitamina E ao lactente. Métodos: Estudo longitudinal observacional realizado com 100 puérperas atendidas para o parto no Hospital Universitário Ana Bezerra (HUAB) da Universidade Federal do Rio Grande do Norte (UFRN), em Santa Cruz (RN). Foram coletados leite colostro (n=100), leite de transição (n=77) e leite maduro (n=63) no seguimento da lactação. O alfa-tocoferol foi analisado por cromatografia líquida de alta eficiência. O fornecimento de vitamina E para o neonato foi estimado comparando-se o requerimento nutricional de vitamina E (4 mg/dia) com a ingestão diária de leite. Resultados: A concentração média de alfa-tocoferol encontrada nos leites colostro, de transição e maduro foi 40,5±15,0 µmol/L, 13,9±5,2 µmol/L e 8,0±3,8 µmol/L, respectivamente (p<0,001). A possível ingestão desses leites pelo lactente forneceu 6,2 mg/dia de vitamina E no colostro, 4,7 mg/dia no de transição e 2,7 mg/dia no maduro (p<0,0001), evidenciando que apenas o último não garantiu a quantidade recomendada dessa vitamina. Conclusões: Os níveis de alfa-tocoferol no leite diminuíram com a progressão da lactação, e a provável ingestão dos leites colostro e de transição conseguiu atender ao requerimento nutricional do lactente. O leite maduro pode fornecer menores quantidades da vitamina E, o que torna importante o estudo dos fatores que se associam a esses baixos níveis.


ABSTRACT Objectives: To determine the alpha-tocopherol concentration in breast milk at different periods of lactation and to estimate the possible supply of vitamin E to the infant. Methods: A longitudinal observational study was carried out with 100 mothers at University Hospital Ana Bezerra (HUAB), at Universidade Federal do Rio Grande do Norte, in Santa Cruz (RN), Northeast Brazil. Samples of colostrum (n=100), transitional milk (n=77), and mature milk (n=63) were collected. Alpha-tocopherol was analyzed by high-performance liquid chromatography. Vitamin supply to the newborn was estimated by comparing the nutritional requirement of vitamin E (4 mg/day) with the potential daily intake of milk. Results: The mean alpha-tocopherol concentration found in colostrum, transitional, and mature milk was 40.5±15.0 µmol/L, 13.9±5.2 µmol/L, and 8.0±3.8 µmol/L, respectively (p<0.001). The possible effect of these milks offered to the infant 6.2 mg/day of vitamin E in colostrum, 4.7 mg/day in transitional milk, and 2.7 mg/day in mature milk (p<0.0001), shows that only the mature milk did not guarantee the recommended quantity of this vitamin. Conclusions: Alpha-tocopherol levels in human milk decrease through the progression of lactation, and the possible intake of colostrum and transitional milk met the nutritional requirement of the infant.
